onsultation by referral and appointment only. No walk in. Referrals can be faxed by MD or NP. The dermatologist will only address the reason for referral.

Hours: 8:30AM to 4PM from Mondays to Fridays
Closed for lunch between 12PM to 1PM

Clinic will be closed on August 4th 2025 and September 17 to 19th 2025

Medical building has dedicated paid parking lot

(only credit/debit cards accepted, no cash).

Please budget a minimum of 1 hour for your dermatology appointment. 

 

It is the patient’s or patient’s representative’s responsibility to keep track of all appointment dates and times. Do not rely on our e-mail notification system.

To ensure timely access for patients with conditions where we can have the greatest impact, our practice focuses on skin rashes and select skin lesions (i.e. undifferentiated, suspicious, premalignant or malignant skin lesions) for diagnosis and treatment.

​

Our practice does not accept referrals for

 

  • hair and nail issues

  • wounds and ulcers

  • warts

  • keloids and scars

  • acne, rosacea, melasma and seborrheic dermatitis (please consider referring to DermCafe.ca)

 

Our practice does not offer cosmetic procedures such as laser, fillers, neurotoxin, chemical peels etc.

Preparing for your appointment

1) Bring a complete list of all oral and topical prescribed medications for all of your medical conditions.

2) Ensure that your OHIP card or other health insurance card is valid for the day/time of your appointment otherwise you may be charged for the medical visit.

3) On site paid parking is available (please budget a minimum of 1 to 2 hours for your appointment as there may be unanticipated clinic delays or you may be offered a same day procedure).

4) Please do not call the clinic if you are running less than 15 minutes late. We will try our best to accommodate late patients.

5) Please do not call the clinic to confirm or verify your appointment. We will see you at your appointment date and time.

Clinic policies 

Only the patient and essential guests (i.e. guests requiring assistance for mobility and/or communication) are permitted in the appointment. â€‹Capable adults are asked to attend their appointments alone.

​

As safety is our priority, we ask that patients make arrangements for the care of their children or dependents during their scheduled appointment time. The clinic team requires your undivided attention. Many patients are offered a same day procedure and we do not have the ability or resources to supervise children or dependents.

​

Patients under the age of 16 should be accompanied by a parent or guardian.

Cellphones, tablets and computers are to be turned off, kept silent or placed in vibrate mode, so as to not disrupt the clinic staff or other patients.
 

No open food or drink is permitted in the clinic.

Avoid applying scented products such as perfume or cologne prior to your appointment. We have many patients with sensitivities and allergies to scents.

We have a zero tolerance policy for any form of abuse, harassment or threatening behaviour. This includes but is not limited to verbal abuse, bullying, intimidation, aggression, derogatory remarks and inflammatory allegations. Offenders will be permanently removed from the practice. 

​

Your patience and kindness is much appreciated. We are a highly specialized clinic, and every hour we see life threatening and severe complex dermatologic cases. We are trying to help and we deserve to be treated with dignity and respect at all times. 

​

Dr. Anthony Mak, MD, PhD, FRCPC, DABD

Dr. Mak is a Canadian and American board-certified dermatologist practicing in Mississauga since 2021. He is a Fellow of the Royal College of Physicians and Surgeons of Canada and a Diplomate of the American Board of Dermatology. 

 

He completed his MD and dermatology residency at the University of Toronto. He was Chief Dermatology Resident at Sunnybrook Health Sciences Centre in his final year of training.

 

In 2023, he practiced as an academic dermatologist at the University of Botswana in Sub - Saharan Africa.  


He also completed a doctorate in cancer biology at the University of Toronto, and was a visiting postdoctoral scholar in stem cell and melanoma research at Brigham and Women’s Hospital (Harvard University) and at Boston University Medical Centre.

No show and late cancellation fees

If you are unable to attend your appointment, please notify us by telephone at least 48 business hours prior to the scheduled appointment day to avoid a fee of $100 + HST.

 

Out of fairness to patients waiting for dermatologic care and the significant administrative costs of the clinic, this fee will be enforced.

Basic Insurance Form Fee  (e.g. prescription medicines)

Completing insurance forms is a non OHIP service and takes considerable clinic resources. The fee to complete a basic insurance form is $30 + HST and can be paid by clicking the "Pay" button to the right. 

​

If you are asking your dermatologist to complete another type of insurance form form (e.g. disability form) then please contact the clinic by telephone.
